How Prolonged Exposure Works

Avoidance is a hallmark of PTSD. After a traumatic event, it is natural to avoid reminders, but this avoidance keeps the fear alive and prevents your brain from learning that the memories and triggers are no longer dangerous.

PE uses two types of exposure: imaginal (revisiting the traumatic memory in a safe therapeutic setting) and in vivo (gradually approaching real-world situations you have been avoiding). Over time, the distress associated with these memories and situations naturally decreases.

What to Expect in PE

1

Understand avoidance

Learn how avoidance maintains PTSD and why approaching memories leads to recovery.

Breathing retraining

Learn a breathing technique to manage anxiety during exposure exercises.

Build your hierarchy

Rank avoided situations to tackle gradually, starting with easier items.

2

Imaginal exposure

Revisit the memory aloud in session and listen to recordings between sessions.

In vivo exposure

Gradually re-engage with avoided places, people, and activities.

Process and reflect

Discuss what you learned after each exposure to replace fear-driven beliefs.

3

Review progress

Compare your PTSD scores from start to now.

Generalize skills

Apply exposure principles to any future avoidance independently.

Celebrate recovery

Recognize how far you have come.
